South Africa’s Nutrition Gap: Why Fortified Peanut Products Matter

South Africa made staple fortification compulsory in 2003. Twenty-odd years on, more than 1.5 million children under five are still stunted, roughly one in four, and the national figure has barely shifted.

That isn’t a failure of the regulation. R504 was written to put eight micronutrients into maize meal and wheat flour, and broadly it does. The problem is that a stunted child is short of more than micronutrients. They are short of calories and of complete protein, and no amount of premix in a cereal fixes that.

Which is why peanut paste keeps turning up in South African nutrition programmes, and why peanut butter products occupy such an odd position in this market: cheap, familiar, nutritionally dense, and at the same time the category that has taken the heaviest food-safety knocks since 2024. Undernutrition and obesity sit side by side here, frequently in the same household. The South African Early Childhood Review 2024 counts over 1.5 million stunted under-fives. A 2025 University of Cape Town survey of 1,214 young children in the Western Cape found 17.5% stunted and 15.1% overweight in the same group. Among adults, WHO-referenced estimates put obesity at 42.9% of women and 18.2% of men.

That pattern is what you get when calories are cheap and protein isn’t.

Regulation R504, in force since October 2003 and amended by R1206 in 2008, requires vitamin A, thiamine, riboflavin, niacin, pyridoxine, folic acid, iron and zinc in maize meal and wheat flour. Its limits are structural:

  • Micronutrients supply no calories and no protein.
  • Maize is low in lysine, so a cereal-heavy diet cannot deliver a complete amino acid profile to a growing child.
  • Retail-level studies have repeatedly found under-dosing, vitamin A in maize meal especially.
  • Every gram needs water, fuel and cooking before anyone eats it.

The delivery network, meanwhile, is already built. The National School Nutrition Programme feeds more than 9 million learners in nearly 20,000 schools, around two in three children in public education. South Africa does not need new distribution. It needs something denser going through the pipe it has.

What does South Africa’s food fortification programme cover?

South Africa introduced mandatory fortification of specified maize meal and wheat flour under Regulation R504 in October 2003, later amended by R1206 in 2008.

The programme requires eight micronutrients:

The South African Department of Health publishes the applicable regulations.

Fortification improves the micronutrient content of staple foods, but micronutrient premixes do not provide meaningful calories or protein. This is one reason nutrient-dense foods can have a separate role in nutrition interventions.

South Africa also has substantial feeding infrastructure. The National School Nutrition Programme provides meals to more than 9 million learners in nearly 20,000 schools (2023/24 figures). Products supplied through institutional programmes must, however, meet the relevant nutritional, allergen, food-safety and procurement requirements.

Why groundnut paste is the standard answer

A fortified peanut base gets round every one of those four constraints at once. Roughly 500 to 550 kcal per 100g from the lipid matrix. Complete protein once blended with milk powder or soy. Low water activity, so it keeps for months with no refrigeration. No cooking, no clean water, no cold chain.

And children eat it, which sounds like a minor point until you have watched a feeding programme fail on palatability.

This is the reason the global standard of care for severe acute malnutrition is a peanut paste rather than a porridge. It is not a preference. It is the only format that carries that much energy and protein into a child in a setting with no kitchen.

    • Ask for aflatoxin certificates of analysis per lot against South Africa’s 10 ppb total aflatoxin limit under Regulation R.1145. Annual facility averages tell you nothing about the pallet in front of you.
    • Check what sorting runs at raw material intake. Optical and density should both be there.
    • Verify FSSC 22000 or BRCGS independently, and check the expiry date.

What is RUTF and how is it different from peanut butter?

Ready-to-Use Therapeutic Food (RUTF) is a specialised peanut-based product formulated for the treatment of severe acute malnutrition, particularly in children aged 6–59 months. Unlike ordinary peanut butter, it contains a defined balance of energy, protein, fats, vitamins and minerals and must meet strict nutritional, microbiological and quality specifications. It is a therapeutic food, not a conventional retail product.

Why import peanut products when South Africa grows groundnuts?

South Africa has domestic groundnut production, but local supply can fluctuate with acreage, weather, crop quality and seasonality. Imports can therefore provide additional volume, specification consistency and supply security. For institutional buyers and private-label brands, the question is less about domestic versus imported product and more about whether the supplier can consistently meet the required food-safety, nutritional, volume and delivery specifications.

South Africa’s fortification programme solves an important problem, but it cannot make a cereal-based diet energy- or protein-dense. That is where peanut-based products earn their place: a relatively small serving can deliver substantial energy, protein and fat in a shelf-stable format that requires no cooking, clean water or refrigeration.

For buyers, however, nutritional density is only half the decision. Peanuts demand tight control of aflatoxin, microbiology, raw-material quality and traceability.
